I acknowledge that participation in VBS at Oakwood Baptist Church involves risk to the participant and/or myself, and may result in various types of injury, including, but not limited to: sickness, bodily injury, and property damage. In consideration for the opportunity to participate in VBS, I acknowledge and accept all the risks of injury associated with participation in VBS for the participant(s) and/or myself. I will not bring my child(ren) if he/she is experiencing any symptoms of illness.
By digitally signing and submitting this Registration Form/Waiver for Oakwood Baptist Church 2023 VBS, I, the Parent or Legal Guardian of the Participant(s), understand that this is a legal agreement that is binding upon myself, and my heirs, executors, administrators, successors, and assignees. I (We) have read and understand the terms of this agreement (above) and I (We) acknowledge that by digitally signing and submitting this form/agreement, I (We) am (are) voluntarily agreeing to abide by its terms, and I (We) am (are) waiving certain legal rights that my child(ren) or I (We) may have.”
I grant Oakwood Baptist Church a license for the following: 1. Use and storage of my image, by means of digital photography and video photography, with respect to VBS at Oakwood. 2. Use of stored data in printed publications, electronic publications, or on the website/social media for Oakwood. (no names /personal info will be used with the photo/video)
